General Information

Please bring the following:

  • Student ID
  • Tribal ID or Certification of Indian Blood
  • Health Insurance card (if applicable)
  • Birth certificate for any dependent children seeking services

Intake Packet – An intake packet must be completed once per school year for any students seeking services within the Wellness Center. The Intake Packet contains client background information, insurance information, a signature of consent.

Eligibility for Services

  • Wellness Counselors – all UTTC students are eligible for services
  • Disabilities Coordinator – all UTTC students are eligible for services
  • Health Promotions Activities – all UTTC students are eligible to participate
  • Food Assistance – all UTTC students are eligible for services
  • Domestic Violence Advocate – all UTTC students and their dependents who are affiliated with a Federally Recognized Tribe are eligible for services
  • Chemical Health – all UTTC students and their dependents who are affiliated with a Federally Recognized Tribe are eligible for services
  • Student Health – all UTTC students and their dependents who are affiliated with a Federally Recognized Tribe are eligible for services

Medical Insurance – all UTTC students and their dependents who are affiliated with a Federally Recognized Tribe qualify for services from the Wellness Center staff free of charge with or without medical insurance. UTTC students who are NOT affiliated with a Federally Recognized Tribe may receive services from the Wellness Counselors and/or the Disabilities Coordinator. If these students are in need of Chemical Health or Student Health Services, they may visit with one of the Wellness Center staff regarding applying for Medicaid or third party insurance for assistance with addressing their health care needs.

Consent for Services – By seeking services at the Wellness Center, consent for services has been implied. Annually, each student seeking services within the Wellness Center will be asked to review and sign a new consent for services form. By signing the consent form you are acknowledging that you understand the potential risks and benefits of seeking services. Questions related to the consent form can be answered by any of the Wellness Center employees. Consent for services may be revoked at any time.

Client Social Media Policy – The Wellness Center staff may utilize the internet and technology in our interactions with you. The Client Social Media Policy clearly defines what you can expect or not expect in relation to staff interactions through the internet or social media.

Cancellation and no-show Policy – No-show occurs when a client has a scheduled appointment with for a service located off-campus (such as with Sanford, Decoteau Trauma, Midwest Vision, etc.) and the client misses that appointment without providing notification 24 hours in advance. A fee of $150 may be applied to your student account if you have “no-show” appointments.

Confidentiality – A medical record is maintained on every person who receives care through the Wellness Center. All of the information contained within your record is kept confidential. Access to the information contained within your record is limited to you (the client) and the Wellness Personnel with a need to know. You may provide written permission for us to release your information to other health care entities. Information within your record will be released in the event of a court order or if there is an imminent risk of self-harm or harm to others. All employees of the Wellness Center sign a confidentiality statement.

Rights and Responsibilities – Students seeking services within the Wellness Center are entitled to certain rights as clients of the center. Students should also be aware that by seeking services they have certain responsibilities while receiving services. If you feel that your rights have been violated in any way, you may voice this concern through the grievance procedure.

Grievance and Appeal Procedure – It is the goal of Wellness Center staff to provide quality services to students at all times. If for some reason a student feels that their rights have been violated or that they have not received the care and services that they desire, the grievance policy will explain the steps to take to make your concerns known.
